Yukon residents enrolling in Community HealthChoices begin with a call to Maximus at 1-877-550-4227. Maximus is the Independent Enrollment Broker for Pennsylvania's mandatory Medicaid managed long-term care program serving adults 21 and older. CHC covers individuals who are dually eligible for Medicare and Medicaid, plus those meeting nursing-facility level-of-care criteria with Medicaid eligibility. A clinical functional eligibility determination confirms the need for long-term services and supports at home. After approval, families choose one of three managed care organizations: UPMC Community HealthChoices, AmeriHealth Caritas, or PA Health & Wellness operated by Centene. Each MCO contracts with different home care agencies and providers, so the selection matters for caregiver continuity. Care Solutions of Pittsburgh participates with each plan and sends personal assistance caregivers to Yukon homes. Our staff support bathing, dressing, transfers, meal preparation, and medication reminders. CHC also funds personal emergency response systems, home-delivered meals, adult day services, and environmental modifications such as ramps and bathroom safety upgrades. The Westmoreland County Area Agency on Aging handles Aging Waiver eligibility under the CHC umbrella for residents 60 and older. Excela Health Frick Hospital in Mount Pleasant manages many local admissions, and our service coordinators align caregiver shifts with hospital discharge plans.
