Community HealthChoices (CHC) is Pennsylvania's mandatory managed long-term services and supports (MLTSS) program. It serves adults 21 and older who are dually eligible for Medicare and Medicaid, or who require a nursing-facility level of care but want to remain in their home and community. CHC took the place of the older Aging Waiver, Independence Waiver, and Attendant Care Waiver — consolidating them under one set of MCOs that coordinate both medical and long-term care benefits.

The Independent Enrollment Broker (IEB) conducts the functional assessment and the County Assistance Office handles the financial eligibility piece.
What home care services does CHC cover?
CHC covers Personal Assistance Services, respite, adult day services, home modifications, Personal Emergency Response Systems (PERS), home-delivered meals, and service coordination. There is generally no copay for waiver services. The exact mix of hours and supports is set in your person-centered service plan.
