Republic families start Community HealthChoices enrollment by calling Maximus at 1-877-550-4227. Maximus serves as Pennsylvania's Independent Enrollment Broker for CHC, the mandatory Medicaid managed long-term care program for adults 21 and older. Eligibility requires dual Medicare and Medicaid status, or Medicaid eligibility plus a nursing-facility level-of-care need confirmed through clinical assessment. Once functional eligibility is approved, your loved one chooses one of three managed care organizations: UPMC Community HealthChoices, AmeriHealth Caritas Community HealthChoices, or PA Health & Wellness from Centene. Each MCO maintains a different provider network, so the choice affects which caregiver agencies and doctors remain in network. Care Solutions of Pittsburgh contracts with each plan and provides personal assistance services in Republic. Our caregivers help with bathing, dressing, transfers, meal preparation, light housekeeping, and medication reminders. CHC also covers personal emergency response systems, home-delivered meals, adult day programs, and environmental modifications including grab bars and stair lifts. The Fayette County Area Agency on Aging administers Aging Waiver eligibility under the CHC umbrella for residents 60 and older. Uniontown Hospital handles most local admissions, and our service coordinators time caregiver visits with hospital discharge plans.
