Houston families begin Community HealthChoices enrollment by calling Maximus at 1-877-550-4227. Maximus is Pennsylvania's Independent Enrollment Broker for CHC, the mandatory Medicaid managed long-term care program for adults 21 and older. Eligibility requires dual Medicare and Medicaid status, or Medicaid eligibility plus a nursing-facility level-of-care need verified through clinical assessment. Once functional eligibility is approved, your loved one chooses one of three managed care organizations: UPMC Community HealthChoices, AmeriHealth Caritas Community HealthChoices, or PA Health & Wellness from Centene. Each MCO maintains a different provider network, so the choice determines which caregiver agencies and physicians remain accessible. Care Solutions of Pittsburgh contracts with each plan and delivers personal assistance services in Houston. Our caregivers help with bathing, grooming, mobility, transfers, meal preparation, and medication reminders. CHC also covers personal emergency response systems, home-delivered meals, adult day services, and environmental modifications including grab bars and stair lifts. The Washington County Area Agency on Aging administers Aging Waiver eligibility under the CHC umbrella for residents 60 and older.
